NO 1. CHECKLIST DOCUMENTS (EASE OF BUSINESS)

•  Application Forms (Form -19 as per Drugs & Cosmetics Act/Rules)
• (a).  Letter of Undertaking
• (b).  Declaration Forms.
• (c).  Appointment Letter for Registered Pharmacist
• (d).  Full time Engagement as Pharmacist
•  Key Plan.
•  Site Plan.
•  Proof of ownership of the premises, if rented (NOC of House Owner)
•  Firm Registration by DC/SDM (Certified Copy).
•  Certificate of non-conviction of Proprietor/Partners/Directors under Drugs and Cosmetics Act, 1940.
•  Certified Copy of Registration Certificate of the Pharmacist (Sikkim Pharmacy Council).
•  Affidavit of Registered Pharmacist/Competent Person regarding fulltime working with the firm duly attested by the Notary.
•  Trade License (Certified Copy)

NO 2. LICENSE FEES

•  Retail License - 3,000/-
•  Wholesale License - 3,000/-
•  Retail + Wholesale (Both) License - 6,000/-

NO 3. TIMELINE FOR THE APPROVAL OF APPROVAL

•  Duration - 15 Days for Retailer + Wholesaler
  (Only applicable for the applicants who register and uploads all the necessary documents)
